Insurance Coverage
Dr. Sharif is currently a contracted provider for United Healthcare, Alternare network (Premera and Lifewise), Aetna, Cigna, First Choice, Regence and Uniform. Regence patients may or may not require a referral letter from their PCP, and should therefore verify this prior to their first visit with Dr. Sharif.
Our office will submit the bill to these insurance companies for you, and you will just pay a co-pay at the time of service. Please call your insurance company ahead of time, and verify your benefits by asking the following questions. It would be very helpful if you bring the answers to these questions to our office with you.
- Is Dr. Sharif a contracted provider for my plan?
- What percentage of my office visits are covered if I see him?
- What is the co-pay per visit?
- What is the deductible for my office visits?
- What percentage of the laboratory tests that Dr. Sharif might order covered by my insurance?
5.1 What if the lab is out-of-network versus in-network? - What is the deductible for laboratory fees?
If Dr. Sharif is not a contracted provider with your insurance company, then ask your insurance company if he would be considered out-of-network? Most insurance providers will reimburse a substantial amount of the visit. You can call your insurance company and ask them the extent of their "out-of-network coverage" for first office call, return visits, as well as laboratory fees.
